Choices Psychotherapy is committed to being mindfully conscience of the environment and to that end, we strive to be 100 % electronic. With 2015 there also came a mandate that all health care providers must have in place an electronic health record system. Choices Psychotherapy is in compliance with this mandate. We realize that not everyone has a computer available to them and we will provide one in the clinic for all clients to complete initial paperwork prior to the first appointment.
At the time an appointment is scheduled, all clients will be emailed an attachment, “client information form.” In the event that you did not receive these forms via email, please contact our office at 952.544.6806 and ask that they be resent. The form will take approximately 20 minutes to complete.
If unable to complete the form electronically, at home, please arrive at your scheduled appointment 20 minutes early to facilitate the completion of the form.
Please note that if you fail to attend your first appointment, we will not be able to reschedule it.
